• Go to Loch Storn (where you fought the Wight Knight in the beginning)
• There you will find a man looking for a weapon with the same name as him Flame Tang
• It is obtainable with alchemy using a Gladius (bought in Stornway) Sunstone x1 ( Lucida shard x2 + Mirrorstone x3 + Hephaestus' flame x1) and Crimson coral x3
• Giving him the dagger completes the quest
